Thank you for choosing Country Companions Veterinary Services for your veterinary needs.  Please download the new client information sheet, fill it out and bring it with you to your first appointment.  You may also mail it in ahead of time so that we can make your visit as stress-free as possible.  Also bring with you a copy of your pets previous medical and vaccination history.  If you do not have it, we can call your previous veterinarian and have them fax it to us.
